accepted as filed and will be returned to you as incomplete
if you do not provide supporting documentation required such
as: A copy of the front page of your Federal 1040 or the Norwalk Alternate 1040 W-2 Forms showing Box 5 and local income tax withheld (Not School District Income Tax) Any federal schedules that relate to business activities including rental property

Tool The City of Norwalk has developed a web based Online Tax Tool, (NOT an online filing program) that is ready for your use in completing the City of Norwalk Income Tax Return. The City of Norwalk Income Tax Department is providing this service to improve efficiency of processing your forms. Completed forms must be printed and mailed to P.O. Box 440, Norwalk, OH 44857 or taken to the City of Norwalk Income Tax Department located at 38 Whittlesey Ave., Norwalk, OH. Your Pin Number is located on the tax form that was mailed to you. If you do not know the Pin Number, you may call the City of Norwalk Income Tax Department at (419) 663-6720. orCity of Norwalk Income Tax Return - Fillable Form Open form and fill in shaded areas that are applicable. Once you have completed the form, press the PRINT button at the top right-hand corner of the page. Sign and date at the bottom and bring or mail the form to the address at the top of the form or use the City's night deposit box. Page 2 detailing business activity including rental MUST be completed manually. Remember, this form is NOT for "electronic filing" and must be delivered to the City of Norwalk Income Tax Department for processing. |
